Best dosage accutane -

Accutane is a great treatment option for this otherwise difficult skin condition. However, it often comes with side effects such as dry eyes, nose and lips, muscle aches, etc. And like a TV drug commercial, I have to mention the less frequent but more severe possible side effects like vision loss, hearing loss, bone growth abnormalities, suicide risks, and GI symptoms. But what if there was a dosage of Accutane that would still clear acne or excessively oily skin or rosacea papules often without the same side effects of the typical dosing?

Sound too good to be true? Normal dose Accutane is regularly dosed based upon body weight and often increases slowly over the course of several months. A low dose regimen uses less milligrams and is taken less often. For example, instead of taking a high dose of 40mg twice a day, the lower dose protocol might be 10mg, twice a week. This off-label use of Accutane provides great control of oiliness, as well as eliminating many, if not all of the common side effects.

This also means we do not know what effects this protocol may have when used long term, or if this eliminates any or all of the more serious side effects. We do, however, still believe that absolute birth control is a must for female patients. If you have persistent acne or oily skin into your adult years, come see us at Dermatology Realm and let us determine if the off-label use of Accutane may be something that is appropriate for your skin.

Experience suggests that females with acne limited to the face are likely to respond to isotretinoin treatment within three months. Males with acne on their trunk or back may take five to eight months to respond to treatment with isotretinoin. Patients with macrocomedonal acne may require as long as 12 to 18 months of treatment with low-dose isotretinoin.

Additional risk factors for a slow response to treatment include: age under 14 years, age over 25 years in females, severe acne, current smoking and polycystic ovary syndrome. Further information and images of macrocomedonal acne is available from: www. The New Zealand recommendations for isotretinoin are based on early studies. These studies, however, only compared doses for a fixed time period. This has led to patients taking lower daily doses of isotretinoin, i. Further information on the unapproved use of medicines is available from: www. New evidence is increasingly suggesting that isotretinoin may be best prescribed using a lower daily dose, with the regimen tailored to the individual patient, the severity of their acne and their response to the medicine. Current guidelines recommend that isotretinoin treatment is calculated based on body weight, usually 0.

There are several problems with prescribing isotretinoin based on a high daily dose to reach a cumulative total amount, including: 3. It is likely that the higher dose group represents traditional weight-based prescribing.

The current total average daily dose of isotretinoin is 42 mg. Research has shown that isotretinoin at doses of 0. Adverse effects of isotretinoin are dose dependent and become more common, and more severe, with higher doses.

The duration of treatment with isotretinoin is currently based on the calculated cumulative dose. This method is used because several early studies suggested that relapse one to two years after a single week course of isotretinoin was more common in people treated with 0. Subsequent research, however, has not supported cumulative dosing. There are no studies that have specifically assessed the most appropriate duration of treatment to clear acne. The current Special Authority criteria for prescribing subsidised isotretinoin recommend that a computer-based decision support tool is used when initiating and renewing the medicine.

The bestpractice Decision Support Module for prescribing isotretinoin has recently been updated to reflect the new research which shows that lower doses are appropriate. The New Zealand recommendations for isotretinoin are based on early studies.

Isotretinoin is recommended for patients with moderate acne that produces scarring or distress, or for acne that persists following other treatments. Low-dose isotretinoin, e. Isotretinoin is an isomer of retinoic acid that has been used for the treatment of acne for over 30 years. Treatment with isotretinoin results in decreased sebum production.

This reduces acne, prevents scarring and may lead to a better quality of life and improved mental health in some patients. Isotretinoin is highly teratogenic and is contraindicated in women if pregnancy is possible unless effective contraception is used Table 1.

Patients with elevated lipids can usually be managed with close monitoring. Advise patients to avoid donating blood during treatment and for at least one month after treatment has finished.

Table 1: Contraindications and cautions for treatment with isotretinoin. To improve absorption and reduce fluctuations in systemic availability, isotretinoin should be taken with or just after food. Low-dose regimens of isotretinoin are generally considered to be as effective as weight-based dosing regimens. A retrospective review of 1, patients treated with isotretinoin over a six-year period concluded that continuing treatment for at least two months after acne had completely resolved was more important than the dosing regimen in determining treatment success.

For additional information, see: www. Previously, treatment with isotretinoin could only be initiated by dermatologists. In , patients began treatment with isotretinoin in the community Figure 1 with approximately equal numbers of patients prescribed low-dose and weight-based treatment regimens.

In , general practitioners became able to prescribe fully subsidised isotretinoin to improve access to people living in less affluent areas. Paradoxically, flares of acne are frequently reported by patients after three to six weeks of treatment with higher doses of isotretinoin. The frequency can then be increased to daily, if tolerated, after four weeks.

Expert opinion is that flares of acne are more common in patients with a large number of macrocomedones facial closed comedones larger than 2 — 3 mm in diameter or very severe acne. To decrease flares, expert opinion is that patients with particularly inflammatory acne can be prescribed trimethoprim, mg per day, for the first six to eight weeks of treatment with isotretinoin.

Patients with acne that is likely to take longer to clear should be advised of this before starting isotretinoin so that treatment is not stopped if results do not occur as quickly as the patient expects.
